$80,000 - $120,000 /yr DOE

  Placement within the wage range for this position is determined by years of experience, qualifications, and demonstrated ability to perform the essential functions.
  Responsibilities
  
  
• Admissions Process
  
• Receive leads from providers, specialty physicians, county offices, facilities, or any customer source and convert appropriate inquiries to admissions.
  
• Coordinate inquiries from all hospitals in the market.
  
• Clinically assess patients to determine if they meet admission criteria, in conjunction with facility requirements/abilities.
  
• Make contact with potential inquiries as a representative of Avalon Health Care. Review charts, and assist with any questions from the case management team, referral source, and/or family members.
  
• Obtain necessary information (medical and financial) to qualify patients for admission.
  
• Facilitate patient placement based on clinical services needed, bed availability, patient/family preference, geographic location, etc.
  
• Troubleshoot areas of Admission, Public Relations, pre-certification, and physician follow-up in conjunction with the facility.
  
• Conduct assessments as needed.
  
• Manage referral sources and relationships in accordance with the facility's census development plan.
  
• Conduct daily visits to referring hospitals.
  
• Build and maintain relationships with Discharge Planners, Case Managers, Physicians, and other referral sources.
  
• Identify new and potential referral sources.
  
• Develop physician relationships through marketing, partnerships, and follow-up.
  
• Maintain thorough knowledge of the market, including products, services, and acuity trends.
  
• Identify options for partnering with other providers to increase facility census.
  
• Coordinate community education and events for hospitals, physicians, and referral sources.
  
• Coordinate tours at facilities for new referral sources and families, and follow through as appropriate.
  
• Follow up with referral sources using the "Marketing the Avalon Way" reporting system, including case studies and updates.
  
• Develop quarterly marketing plans, updating them monthly.
  
• Submit weekly reports and goals as required.
  
  Qualifications
  Education / Experience
  
  • 
    
  • RN or LPN license in good standing strongly preferred.
    
  • Associate and/or Bachelor's Degree in healthcare, preferably Nursing.
    
  • SNF or hospital discharge planning experience preferred.
    
  • 1-3 years of clinical nursing experience, with at least two years in long-term care and two years in administration, consultation, or survey process involvement.
    
  • Customer service, sales, and marketing experience preferred.
