After-Hours Registered Nurse (7 Days On / 7 Days Off) At Elara

Impact as an After-Hours RN As an After-Hours Registered Nurse, you
will be responsible for assessing, coordinating, and delivering
patient care during evenings, nights, and weekends. Your role
includes: - Performing initial assessments and follow-up
evaluations for a home health patient caseload. - Identifying
physical, emotional, and environmental care needs and developing
safety-focused care plans. - Communicating critical updates to the
clinical supervisor, physicians, and care teams. - Responding to
changes in patient condition and initiating appropriate nursing
interventions. - Implementing and verifying physician orders in
line with individualized care plans. - Maintaining accurate and
timely clinical documentation. - Participating in case conferences
and collaborating to ensure continuity and quality of care. -
Performing other duties and special projects as assigned. What
You’ll Need to Succeed - Graduate of an accredited nursing program
(Certificate, Diploma, Associate, or Bachelor’s) - Current RN
license in the state of practice - At least 1 year of clinical
nursing experience (home health preferred) - Willingness and
ability to travel approximately 50% - Strong critical thinking,
communication, and documentation skills - Compassion, reliability,
