Your role includes: - Performing initial assessments and follow-up evaluations for a home health patient caseload. - Identifying physical, emotional, and environmental care needs and developing safety-focused care plans. - Communicating critical updates to the clinical supervisor, physicians, and care teams. - Responding to changes in patient condition and initiating appropriate nursing interventions. - Implementing and verifying physician orders in line with individualized care plans. - Maintaining accurate and timely clinical documentation. - Participating in case conferences and collaborating to ensure continuity and quality of care. - Performing other duties and special projects as assigned.
