Pay Information $3,233 per week About The Position Job Title: Case Manager Profession: Registered Nurse Specialty: Home-Hospice Care Operations Duration: 13 Weeks Shift: 8 Hour Day Shift Hours per Shift: 40 Experience: 1 Year Home Health RN Experience License: Active State RN License Certifications: Active BLS (AHA) Must-Have: - Wound care experience (wound vacs, pleural drains, and ostomy care) - Able to complete peripheral blood draws, central line care, and IV administration through multiple devices - Excellent assessment skills - Experience with computer documentation - Reliable transportation, active driver’s license, and insurance Description: Traveler can expect to be onsite for orientation for approximately 2 days. The position is 100% remote within the designated coverage area. There is no physical office to report to; all meetings are held virtually. Supplies and equipment will be delivered to the traveler's home. Candidate must provide their own transportation to/from patient visits. Case Manager can expect to see 20-30 patients per week. The expectation is for 6 routine visits per day. Patients typically have multiple comorbidities, which may include conditions such as CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, falls, fractures, infections, cellulitis, post-surgical care, wound care, and ostomy care. The role involves providing education on medications, disease management, and self-care. Candidates will also perform skills such as wound care, infusion, line maintenance, and ostomy care. Teaching patients how to perform their own care is also a key responsibility. Computer charting will be conducted using EPIC. Sapphire IV pumps will be utilized. Any solid color scrubs are acceptable. Candidate must provide their own transportation to/from patient visits.
